What is BHRT?
Bioidentical hormones are structurally identical to the hormones your body already produces — most often estrogen and progesterone. BHRT is used to ease symptoms tied to hormonal changes and to protect long-term health where hormones have dropped earlier than they should.
In Ontario, naturopathic doctors who've completed the required regulatory certification can prescribe bioidentical estrogen and progesterone in topical or suppository form. In practice, this means precise, individualized dosing designed to work with your body — paired with the testing and follow-up needed to know it's actually helping.
Who BHRT can help — it's not just about perimenopause
Hormone therapy gets talked about almost exclusively in the context of perimenopause, but that's only part of the picture. It's also an important option for:
Perimenopause and menopause — the most common reason women come in, as estrogen and progesterone fluctuate and then decline.
Premature ovarian insufficiency (POI, sometimes still called POF) — when the ovaries stop functioning normally before age 40. Research consistently shows hormone therapy is underused in this group, even though these women lose hormonal protection much earlier than average and often benefit the most from replacing it.
Early or surgical menopause — including after an oophorectomy (ovarian removal) performed before natural menopause age. The sudden loss of hormones here can be more abrupt and pronounced than a gradual perimenopausal decline, and therapy is often recommended at least until the typical age of natural menopause.

BHRT isn't for everyone
Hormone therapy has real, well-documented benefits for the right candidate — but it's not automatically appropriate for everyone, and part of my job is helping you figure out honestly whether it's right for you. It generally requires a closer look (and sometimes isn't recommended) if you have a personal history of breast or uterine cancer, unexplained vaginal bleeding, blood clots, liver disease, or cardiovascular disease. Timing also matters: starting closer to the onset of menopause is generally associated with a better benefit-to-risk balance than starting many years later.
None of this is meant to be a diagnosis by checklist — it's the reason a proper history, testing, and an unhurried conversation come before anything is prescribed.
How I approach BHRT differently
Hormone therapy isn't the first or only conversation worth having, and dosing isn't one-size-fits-all. My approach looks at hormones alongside gut health and metabolic function, because these systems influence each other constantly — a hormone imbalance is rarely happening in isolation.
That means:
Real testing, not guessing. We look at your hormone levels, symptoms, and history before deciding whether BHRT is appropriate for you.
A plan built around you. Dosing and delivery are adjusted based on how your body responds, not a standard protocol.
The full picture. Sleep, stress, digestion, and metabolic health are part of the conversation, because they affect how well hormone therapy works.
Ongoing follow-up. We reassess and adjust — this isn't a prescribe-and-forget approach.

Current guidance supports its use within an individualized risk-benefit framework — it's a genuinely good option for many women, and genuinely not the right choice for some. A proper history and testing come first, every time.
Is BHRT only for perimenopause or menopause?
No. It's also an important option for premature ovarian insufficiency (POI/POF) and early or surgical menopause, where hormone loss happens well before the typical age — and where therapy is often especially valuable for long-term health, not just symptom relief.
Is BHRT the same as compounded hormones sold as "custom" or "natural"?
Not necessarily — the term gets used loosely. "Bioidentical" refers to the hormone's molecular structure matching what your body makes; it doesn't automatically mean unregulated or untested. Ontario naturopathic doctors prescribe within a regulated framework, using pharmacy-dispensed topical or suppository preparations.
